Service Requests for both Voice (telephone) and Data (network access) are submitted using a Telephone Service Request (TSR) Form.
Service Order turnaround time to complete the work is typically 5 business days.
Select the type of action for the service you require:
- Add a Service
(e.g., need a new phone line or jack installed, speakerphone rental)
- Delete a Service
(e.g., phone line no longer needed)
- Change a Service
(i.e., telephone feature change)
(e.g., change stations' billing account number)
(e.g., change single-line to business set)
(e.g., purchase phone equipment)
- Move a Service
(e.g., moving into a new office or building)
(e.g., need a dedicated 800 Conference service)
- Data Activation
(e.g., need an existing data/network jack activated)
- Online Phone Bill Access
(e.g., change recipient information for phone bill)
- Voicemail Service
(e.g., reset password on voicemail)
- Virtual Telephone Number
(VTN information here)
In order for your Telephone Service Request to be processed, you must:
- complete all required/relevant areas of the form and press the CONTINUE button to produce a printable form with a unique TSR number
- print the TSR form from the resulting Web page
- have the dean/division head sign the form
- fax the completed form to IT Customer Services at 313-577-5577*
*Send Medical School forms to Facilities & Support Services (1102 Scott Hall) or fax them to (313) 577-2466. Do not send them directly to IT Customer Services.
